Here is a sample book proposal form. You will need to be able to answer all these questions thoroughly. The main difference between fiction and non-fiction is with fiction you will need a completed, polished manuscript. With non-fiction an outline with subtopics and first two-three chapters is fine to be offered a contract. Even if self-publishing, you should still complete this.
